Despite having undergone major procedures like surgeries or laser treatments, the thought of a dental appointment still elicits a unique level of dread in many people. Dentistry is, therefore, one of the most intricate and challenging medical fields, demanding not only technical precision to work in the small confines of the mouth but also the emotional skill to assuage the intense fear and apprehension many patients have. In this edition of MedPark Stories, we sit down with Dr. May or Dr. Vimonkan Damrongratnuwong, a dentist specializing in periodontics, to explore the fascinating and surprising aspects of the dental profession.

Journey to the Dentistry: Making the Impossible Possible

“Is this truly possible?” The question, sparked by astonishment and newfound knowledge, inspired Dr. May to pursue a career in periodontal dentistry.

“From the moment I began preparing for my entrance exams, I was firmly committed to a career in medicine. Dentistry stood out as my top choice because of its flexibility. I can avoid night shifts, manage my work schedule, and have a perennial career while still having time to prioritize my well-being.”

“Early in my career, I attended an academic conference where I was profoundly impressed by the presentations of esteemed professors. I witnessed how dental bone grafting could be achieved and effectively reverse conditions where teeth were on the brink of extraction. Teeth that once appeared beyond recovery were restored to health and preserved. I realized periodontists could accomplish the impossible, giving patients hope they never thought possible. On that day, I decided to specialize in this field—it ignited a passion within me.”

Dr. May revealed periodontics are a perfect fit for her once she began working in the field. In this role, she engages in insightful conversations with patients, delivers effective treatments, and provides crucial information they may not have been aware of. When her interventions result in remarkable improvements -- transforming patients from pain and dietary limitations to a state where they can fully enjoy their meals— She believes she has achieved her professional goals and aspirations.

Addressing Patient Fears Requires Patience

“Every medical specialty has its unique challenges, and for dentists, one of the most significant is managing patient fears.”

Dr. May explains that it is entirely normal for patients to experience fear or anxiety in a dental setting. The sounds of instruments, the intrusion of dental tools in the mouth, trouble breathing, and the need to keep their mouths open for extended periods can all contribute to unavoidable discomfort and stress.

“I had a patient tell me that childbirth was less frightening than a dental visit. Some patients may not verbalize their anxiety, but their nonverbal cues, from my experience, are indicative of their fear. I consistently reassure patients that if they feel anxious or uncomfortable, they have a way to convey it to me. Fear is a natural response. They are encouraged to raise their hands to signal their discomfort during the procedure.” 

Dr. May begins each appointment by assessing what needs to be addressed and discussing it with the patient. She initiates treatment with incremental steps, gradually advancing as tolerated. If a patient can only handle part of the treatment during a session, she will pause and schedule the remaining work for a future visit.

“Will patients still feel anxious at their next visit? It is possible, but by then, they will have a clearer understanding of the procedure and see that it is not as intimidating as they initially thought. This reassurance helps them to complete their treatment more comfortably.”

Dr. May said she has to work against the clock, striving to minimize the time patients need to keep their mouths open. It requires careful planning and attention to ensure that both immediate and long-term outcomes align with the expectations and trust patients place under her care.

Periodontal Care: A Commitment for Rewarding Outcomes

Periodontics requires significant understanding and cooperation from patients. Dr. May recounts a particularly memorable case:

“I once treated a patient with severe gum swelling, a burning sensation in the mouth, loose teeth, and recurrent abscesses. Despite multiple treatments at various clinics, the condition persisted, leaving the patient deeply worried and uncertain about their diagnosis. After examining X-rays and detailing the treatment plan, I explained the necessary procedures, the number of visits required, and the anticipated outcomes at each stage of the course of treatment.”

“When the treatment yielded positive results—with loose teeth and abscesses resolved—everyone was thrilled. Patients valued my meticulous care, with some referring family members praising my attention to detail. I feel great satisfaction. Early detection and intervention in oral diseases offer the best treatment outcomes.”

Rest Days Dedicated to Self-Care for Optimal Patient Care

“My hobby aims to prioritize self-care to maintain my ability to continue delivering exceptional care.”

In discussing her leisure activities, she notes that, given the scarcity of periodontists, even a brief absence can result in extended patient wait times. As a result, she prioritizes her health and well-being.

”During my free time, I typically rest, engage in Pilates, and stay updated with the news. I also enjoy exploring new restaurants when time permits. These activities provide me with great joy and balance.”

Before concluding the discussion, Dr. May provided insightful recommendations on oral care that many may overlook.

“Oral health care is unique to everyone, necessitating personalized care approaches. Habits and dietary choices can result in subtle yet significant tooth wear and damage. Inefficient mastication and inadequately chewed food can impact overall well-being.”

“One can appreciate individual nuances only through a comprehensive examination to evaluate the severity of the issues. It is essential to be diligent about oral hygiene and to regularly consult a dentist to establish a tailored and effective care plan.”

Addressing patient anxiety involves empathy and patience; progress is gradual. Dentists can help patients overcome these challenges to achieve the most satisfactory treatment outcomes by fostering a supportive environment.
